#PelAir – 18th November 2009 – A short review

The sixth anniversary of the #pelair ditching has come and gone.

#atsb says:

During the afternoon of 11 November 2015, the rear section of VH-NGA, which contained the flight recorders, was lifted onto the deck of PMG Pride. Both the cockpit voice recorder and flight data recorder were removed from the wreckage and placed into sealed containers in preparation for transportation to the ATSB’s technical facilities in Canberra.

The part numbers and serial numbers of the recorders agree with the maintenance documentation for the aircraft. The recovery and storage of the recorders was witnessed by an officer of the Australian Federal Police. Examination of, and data recovery from, the recorders is expected to commence during the week of 16 November 2015.

I attach a couple of the important #ASRR findings and say that the industry is not seeing #casa compliance.

17. The Civil Aviation Safety Authority publishes and demonstrates the philosophy of ‘just culture’ whereby individuals involved in a reportable event are not punished for actions, omissions or decisions taken by them that are commensurate with their experience and training. However, actions of gross negligence, wilful violations and destructive acts should not be tolerated.

In fact, there is no move this way and #casa is not subject to the penal provisions of the Public Service.

22. The Civil Aviation Safety Authority establishes small offices at specific industry centres to improve monitoring, service quality, communications and collaborative relationships.

The latest is the pending closure of Townsville and rationalisation of  Cairns.

30. The Civil Aviation Safety Authority changes the current two-tier regulatory framework (act and regulations) to a three-tier structure (act, regulations and standards), with:
a. regulations drafted in a high-level, succinct style, containing provisions for enabling standards and necessary legislative provisions, including offences
b. the third-tier standards drafted in plain, easy to understand language.

Distinctly not being done and no plans to look at Part 61 [for example] in this light. If #casa did, we may end up with reasonable regulations.

Pel-Air 2nd final report now has data, and a stab at a date

The disgraceful saga of the ATSB inquiry into the ditching of a Pel-Air Westwind jet in 2009 now has black box data, and an indication of a date, for its second and forced final inquiry into the accident.

This update has been posted on the ATSB website:

Data from both the CVR and FDR from VH-NGA has been successfully downloaded at the ATSB’s technical facilities in Canberra. The CVR data is of just over 2 hours and 4 minutes duration, covering the period up to the aircraft’s ditching into the waters off Norfolk Island. The FDR contained 116 hours of data, which included the last four flights of VH-NGA, which were undertaken in the period between fitment of the particular recorder to the aircraft on 3 November 2009 and the date of the accident (including the accident flight). The evidence obtained from the recorders will be analysed as part of the ongoing investigation and relevant information will be included in the ATSB’s final report, which is expected to be released in the first quarter of 2016.
